Key Responsibilities
- Educate members on healthcare options, benefits, and services.
- Support cancer diagnosis, treatment coordination, survivorship, and supportive care.
- Conduct clinical assessments, prioritise needs, and guide appropriate care.
- Refer members to specialists, expert medical opinions, and complex case management.
- Coordinate virtual primary care, diabetes management, physical therapy, behavioural health, and other programmes.
- Provide evidence-based health education and follow-up support.
- Maintain accurate clinical records and comply with HIPAA and applicable privacy requirements.
- Collaborate with multidisciplinary teams and support continuous improvement.
Qualifications & Requirements
- BSN or equivalent nursing qualification.
- Active, unrestricted RN license and active Nurse Licensure Compact (NLC) license or applicable eligibility.
- Minimum 3 years of recent direct patient care experience.
- Strong clinical assessment, triage, critical-thinking, communication, and digital skills.
- Oncology, telephonic triage, medical-surgical, maternity, emergency, behavioural health, or home healthcare experience preferred.
- Knowledge of medical benefits, insurance claims, and care coordination advantageous.
